NCHHHA Receives Standard of Care Recognition

Littleton, NH – February 13, 2026 – North Country Home Health & Hospice Agency (NCHHHA), an affiliate of North Country Healthcare, recently received the results of a January 2026 Centers for Medicare and Medicaid Services (CMS) survey assessing how NCHHHA delivers care, ensures safety, and complies with regulations. These annual, unannounced surveys are comprehensive audits that provide a snapshot of how an Agency is performing and the quality of care they provide. NCHHHA is proud to share they received deficiency-free survey results for both their home health and hospice services.

A deficiency-free survey is a major achievement: most agencies will receive at least one deficiency. Some deficiencies are minor, like documentation errors, while others are more serious, like patient safety concerns. A deficiency-free survey means CMS found no violations, provided no citations, and required no corrective action plans. A deficiency-free result reflects a well-run Agency with exceptional attention to detail, consistent staff performance, a strong culture of accountability, and, above all, dedication to providing quality patient care.

NCHHHA provides care to patients in their homes in Grafton and Coös Counties, from around Plymouth, NH north to the Canadian border. The NCHHHA caregiving team includes medical providers, clinical support staff, social workers, homemakers, spiritual counselors, and trained volunteers. Home care can include help with daily caregiving tasks, symptom management, prescription adjustments, and 24/7 availability for care needs or questions. NCHHHA’s care teams help patients focus on quality of life and meeting important personal goals.

NCHHHA offers both home health care and hospice care. Home health care can help patients avoid repeat hospital visits by treating illness or injury at home, or help patients remain independent in their homes. Referrals for home health care can come from a community organization or a patient’s healthcare provider. Hospice care helps patients and families cope with end-of-life concerns, such as symptom management, pain control, and emotional support. Hospice referrals can come from providers, families, or the patient themselves.
